Any owner may obtain a dog or cat license by making a signed written application to
the city manager or the manager's designee. The application shall contain the name,
sex, age, breed and description of the dog or cat and the name and address of the
owner, and shall be accompanied by the amount of the license tax and a certificate
of vaccination or other evidence from a licensed veterinarian showing that the dog
or cat is inoculated against rabies and the expiration date of the vaccination. Upon
receipt of a proper application, accompanied by the amount of the license tax and
a proper certificate of vaccination or other evidence, the city manager or the manager's
designee shall issue a license receipt and an appropriate tag to the owner, and shall
record upon the receipt and upon records maintained by the city the name and address
of the owner, the date of issuance, the period of time for which issued and date of
expiration of the rabies vaccination, the serial number of the license and the sex,
